Faith House Surgery

We are an N.H.S. General Medical Services practice in Hull, Yorkshire, UK and also a teaching practice linked to Hull York Medical School (HYMS) for 4th year medical students.

There are 5 doctors in the practice, 2 of whom are part time. As a team, we aim to work as partners with you, our patients, in caring for your health. We will do our best to diagnose illnesses and treat where appropriate and encourage you to live a healthy lifestyle. However, in the end, your health is your responsibility.

As well as face to face appointments (on the same day if needed) we also offer the option of speaking to a doctor on the phone.

BOOKING APPOINTMENTS ON LINE:
You can book, and cancel, doctors appointments on line, for up to 8 weeks ahead for routine appointments. There are also some embargoed pre-bookable routine appointments and same day appointments which become available for you to book from 6.30pm the previous working day, thus saving worry about whether you can get through on the phone. You will also be able to view and request repeat prescriptions though this service. Please contact reception to register for this service.

SURVEY FEEDBACK:

The results of the 3 monthly rolling GP patient survey of General Practices is available on a national website: http://results.gp-patient.co.uk/report/9/result.aspx - this should take you straight to the search page. If you end up on the home page click on link to "Detailed Practice Data and Toplines" box to take you to the search page, then type in the search box: "Faith House". To compare us with other practices from the Home Page follow the instructions above but type in the search box "Kingston" and look at any of the Kingston-upon-Hull practices. The survey is posted to a random sample of patients every 3 months, so please complete it if you receive one. You can also compare our practice with the Hull PCT average, the Yorkshire & Humber area average and with the National average.

We value patients views so please write to the Practice Manager with praise when you think we are doing well or express your concerns when we are not doing so well. Please also express views on the NHS Choices website - it is important that positive views as well as negative views are expressed in order to reflect what is really going on. Thank you.
